Insurance Information
We are contracted with many Medical Groups and California Insurance Plans.
Our list changes frequently. Please contact your insurance company to verify
elibility prior to scheduling an appointment.
If we are not on the roster of your insurer, consider keeping your appointment.
Upon request, you will be provided a copy of the invoice for the visit (for which
we can accept MasterCard, Visa, or most debit cards). You may then submit for
reimbursement to a non-contracted insurer.
Below is a list of current accepted Medical Groups and California Insurance Plans.
Feel free to speak with a benefits advisor (by calling the 1-800- number on the back
of your insurance card) to verify that Dr. Haskins is currently contracted with your
plan, as changes can occur without notice. Give them her name and her EIN (26-1908524)
to make this verification.
Be aware that we only can accept a limited number of pregnant patients per month (otherwise
the quality of our care would suffer) and we can only accept a limited number of patients
from some insurance plans, otherwise our reimbursements would not permit us to continue caring
for all the patients we have accepted in our practice.
